തൃശ്ശൂര്‍: രോഗിയായി ഭാവിച്ചെത്തിയ ശേഷം ഡോക്ടര്‍മാരെ ഭീഷണിപ്പെടുത്തി പണംതട്ടുന്ന ആള്‍ ജില്ലയില്‍ വിലസുന്നു. പതിനഞ്ചോളം ഡോക്ടര്‍മാരില്‍നിന്ന് ഇയാള്‍ പണം തട്ടിയിട്ടുണ്ടെന്നാണ് ഡോക്ടര്‍മാര്‍തന്നെ പറയുന്നത്. എന്നാല്‍ പലരും ഇത് പുറത്തു പറയാന്‍ മടിക്കുന്നു. ഹോമിയോപതി ഡോക്ടര്‍മാര്‍ സംഘടനാതലത്തില്‍ ഡിവൈ.എസ്.പിക്കു പരാതി നല്‍കാനിരിക്കുകയാണ്. കാഞ്ഞാണിയിലെ ഒരു ഇ.എന്‍.ടി. ഡോക്ടറെയും ഇത്തരത്തില്‍ കബളിപ്പിച്ചിട്ടുണ്ടെന്ന് സംസാരമുണ്ട്.

A CURED CASE OF SLE By Dr R SARATH CHANDRAN

Miss. Aparna consulted me on 10^th May 2010. She had been under the treatment of department of rheumatology Medical College Thiruvanathapuram. Final diagnosis of Rheumatology Department Thiruvananthapuram in this case was SLE with parenchymal renal disease.  Her case was taken in detail and we got the following details.

She was born in a poor family as the 2^rd daughter of her parents. Her mother was a part time sweeper in animal husbandry department. Her father was absconding when she born and returned after 10 years i.e at the age of 9 of Aparna. Their life became more troublesome after the arrival of her father. He was a drunkard and was a suspicious husband. His teasing and harassments made Aparna’s child hood very much miserable.  As her age advances she started quarreling and even fighting with her father to protect her mother. Survival of her mother, one elder sister and a new born kid aged 6 months was depending up on the courage and fight of Aparna at her 13^th year of existence in this world. Though things are like these Aparna studied well and get out of each examination with high marks.  Her menarche was at the age of 13^th. And her health problems started then and there. There were repeated episodes of joint pains and brownish discoloration on face during menses. She was not able to look in to her health issues as her mother was too week to move due to the concussions and contusions in her mind and body that received from her husband. To add fuel to fire Aparna’s father fell ill due to tuberculosis. He was admitted in a T B asylum to protect others from TB. Her father died after one year and mother also left her soon after the death of father. Aparna’s elder sister was taken cared by an orphanage but Aparna didn’t go. She took the responsibility to look after her younger sister but for that she sacrificed her studies for one year.  By sharing a small room in her uncle’s house she started studying after one year and has crossed SSLC with a distinction. Meanwhile she got a small job in a press run by the orphanage where her elder sister stays. She manage her plus 2 studies and part time job very well and send her younger sister to LKG then UKG and so on. But unfortunately the disease grows inside her system to make her unable to continue studies or even mange her job in press. Orphanage authorities shifted her to the local hospital and she got relief of pain and swelling of joins that recurred again and again. Finally she was investigated thoroughly at Thaluk hospital and referred to medical college hospital. She was admitted in Medical College Hospital Thiruvananthapuram on 8^th November 2008 and was treated by specialists who put her under steroids. She was discharged from MCH from nephrology department with a discharge summery shows SLE, Lupus Nphropathy! (See Lab result – 1).  She was doing well with steroids initially and changed her job from press to a tailoring shop. Very painfully she realized that she cannot continue studies because of financial crisis as well as health crisis. She gave up her ambition to become a BSc nurse. In her words it was the most painful reality of her life. But she continued studies even without going college. She took BA in psychology through distant education. She found pleasure in her job and enjoyed it. She herself starts a new tailoring center and was very particular in her job. Though she fell in negative thoughts of her past bitter experience she was active and cheerful while doing her job. She wants to earn money to secure the future of her family.  She doesn’t have any friends and she doesn’t want to face them. She kept in home and enjoys tailoring and earns money. But her happiness ends with in a year. She became fatty due to continue usage of steroids and her kidney damaged and fell in to a worst form of SLE. Her life has been totally changed due to the last episode of SLE. She has been changed both mentally and physically. She lost her confidence and fighting instinct. She transformed from an optimist to a pessimist. She gave up her ambition to make money and to save her sisters from poverty. She doesn’t want to move even and refused to take medicines as it worsens her condition.  She was forcefully admitted in Medical College Hospital that further worsened her condition. She was discharged on her request and orphanage authorities who had been taking care of her. Orphanage authorities told her about Homoeopathic treatment. Because of their repeated request she agreed for treatment. She herself narrated her story and was weeping in between. Two to three times she stopped her story because of her tears.

Just after going through the discharge summery our team of doctors examined her. She was anemic; there was yellow coating of tongue and yellow discoloration of sclera.

Respiratory rate	26/minute

Very few questions were asked as she drew her picture very well. She likes sweets and her menses was irregular or absent since steroids started. Perspiration only on face and she was sleepless because of pain.

Her case put on analysis immediately as she was suffering than anything. Primary conclusion and existing miasmatic ratio was as follows.

There was lots of suppression in mental level due to her childhood circumstances and in physical level due to steroids. All suppressions come under sycotic miasm. Now she is in a degenerated state mentally and physically. SLE itself is a degenerating disease so prominent miasm in existing combination is syphilitic.

So she is in need of a Syco-syphilitic remedy.

It can move even without water and it won’t struggle when it thrown out of water. So the nearest Fish SEPIA was selected and was given in 1M potency as and when required. She might have received 6 to 8 doses within a matter of 8 months. She brought back to life with her old vim and vigor. Instead of going back to tailoring she herself started a tailoring Institute and taught poor ladies how to stich and how to make money out of stitching. Painful 5 years in her life made her bold enough to fight for survival again. After withdrawing steroids she became slim like a fish. Sepia made her symptom free and tension free so she was advised for an ANA & Anti Ds DNA on 1^st February 2011 and found negative. (See Lab Result 2 & 3) As she is symptom free as well as disease free she is advised to discontinue treatment on2/02/11.

Miasmatic Interpretation of the story

Sweat on face as droplets cum under syphilitic miasm. വലിയ നിറയെ മുത്തുകള്‍ പതിപ്പിച്ച മോതിരമിട്ട കൈകളാല്‍ '-it is a show off. And it comes under sycotic miasm. She is sitting by crossing her limbs-this shows that she is dominating character. Again it comes under sycotic miasm. ‘വലിയ കമ്മലിടാന്‍ കഴിയുമായിരുന്നിട്ടും ചെറിയ മുത്തുകള്‍ നിറഞ്ഞ കമ്മലിട്ട കാതുകളിലൂടെ കേട്ടപ്പോള്‍'-this sentence shows that she is having big ear lobe again sycotic miasm. ‘സാവധാനം ചിന്തയില്‍ നിന്നും ഉണര്‍ന്നു '- sluggishness of mind, again comes under sycosis. She likes lays that is salty pungent and spicy thing hence again sycotic miasm, and also Lays are a show than tapioca chips, comes under sycotic miasm. ‘കണ്ണുകളില്‍ പ്രകാശം വിടര്‍ത്തി ‘-this represents her excitement when she saw lays. This is psoric feature.  She looked upon the beggar. But when another person gave the beggar a coin she gave 10rs just for the sake of showing off, again sycotic. The beggar who earns money just for his day to day life, a psoric character.

They never enter in to the intimate zone of others in physical and mental level. 75 centimetres around a person is considered as his intimate zone. Only a very close person shall enter in to your intimate zone for communication. Aloof person never enter in to this zone. He hesitates to shake hand with you without an invitation. He never asks your personal matters and even personal details. One of my friends in Jaycees (An international youth organization) doesn’t know the whereabouts of his fellow members. Once he was appointed as the programme director of an event. His duty is to call every member over telephone and to welcome the gathering. He literarily failed in these 2 duties because he doesn’t know what his fellow members so as to guests! Remember he was the winner of 100% attendance Award in the previous year. He presented in all meetings attended all training programmes but keep a distance from his fellow members! In another occasion one of the member of the same organisation brought all members and beautifully introduced not only the guest but also all members! He introduced each every member of our organization by narrating an event in their life and their reaction to that event. This introducer was a part of each event! He took initiative in others matter and played his role without anticipating a negative outcome. He made certain condition worsen because of his interference. When both these patients consulted me on different occasions I take Aloof for former and Meddlesome for the later.

CHARLATAN: A person who makes elaborate, fraudulent, and often voluble claims to skill or knowledge; a quack or fraud. A retired air force officer told me that he is an encyclopaedia in aviation. Whenever he reached in my clinic he narrated something that I couldn’t get in terms of aviation. He uses technical terms of aviation. He knows only aviation and what he told was aviation always. I took his rubric as boaster, flattering desire to be. Etc. After few months I purchased a new car. It was written on the dickey of the car that “Quadra jet”. I hesitate to ask Car Company about the meaning of “Quadra jet” and I thought that I can ask aviation man who is well versed with engines and all as he stated. Meanwhile I got information that my new car’s is engine is “turbo” that too I don’t know what is. I kept both questions for aviation man. When he reached I just asked him both questions. He was literally hunting for words, he don’t know the answer of both questions. Later I invited him for a speech in the nearby school in connection with Independence Day organized by Rotary International. He skulked from that invitation by saying some unbelievable excuses. I called him for another occasion for a speech about aviation in Rotary club. At that time I told him to choose the date and time but till date he postponed it even after my repeated request. It was actually a fraudulent clam about his aviation knowledge! All wars are against Humanity. If a war started in human body it is called autoimmune diseases. The Body has its own Immune system to fight against the germs, pathogens and organisms that attack body. But what will occur when this immune system, that is the soldiers of Human body attack tissues and cells?, autoimmune disease is such a state.  Auto immune diseases are first identified by a Japanese researcher Hashimoto Hakaru. He found the autoimmune nature of thyroiditis and it is called as Hashimoto thyroiditis. The latest studies are shown that many diseases which have called as unknown etiology, has a connection with autoimmune mechanism. Many researchers close their research work with adding a new disease to the list of autoimmune diseases!

Patients suffering from auto immune disease are submitted for immune suppressive therapy. Main drawbacks of immune suppressive therapy are recurrent infection, loss of bone marrow, osteoporosis, organ damages especially liver, kidney and pancreas. Alopecia areata

Mainly alopecia areata, universalis, alopecia  totalis, alopecia areata barabae only affecting beard, etc. are common clinica presentation. In alopecia the Immune mechanism work against the follicles of hair. The peculiarity is that it will affect only black hair follicle, so thought that autoimmune action related with melanin production mechanism. In alopecia areata there is small black spots seen in hair loosed area, it is due to the destruction of hair in hair follicle while its formation.

Ankylosing spondylitis

This is the most common chronic arthritis seen. It is the chronic inflammatory arthritis of autoimmune nature mainly affecting spine and sacroiliac joint. Complication of this disease includes fusion of spine, bamboo spine. It is a gradually appearing disease. Most important problem in AS is diagnosing because average onset to diagnosing lag time has been estimated to be approximately 8.5 to 11.4 year. In AS X-ray is the commonly using diagnostic technique. But the bone change that appears in X-ray will take 8-10years to develop. That means its signs and symptoms will start showing years back before its changes appear in X-ray. Another diagnosing technique is HLAB27 gene, which is positive in 90% of AS Patients, but it will be positive in 5% of normal people too. Like other inflammatory diseases C-reactive protein and ESR will be increase in acute phase.

Auto immune hepatitis

Autoimmune hepatitis is less common. In autoimmune hepatitis Anti-Nuclear Antibody  (ANA), anti-Smooth Muscle antibody, Liver-Kidney Microsomal antibody (LKM-1,LKM-2,LKM-3), anti-soluble liver antigen and Anti-Mitochondrial antibody will be present in Blood.

Diabetes mellitus type 1

In autoimmune diabetes mellitus immune system work against beta cells of pancreas and cause insulin deficiency, there by develop diabetes mellitus. Anti-Insulin Antibody & Anti Islet cell antibody will be positive in auto immune DM.

Good pasture's syndrome

This is an autoimmune disease characterized by glomerulonephritis and hemorrhaging of lung.Good pasture’s antibody cause this autoimmune disease. This antibody present in kidney and Lungs.

Graves' disease

Here autoantibody (TSHR-Ab) activates TSH receptor of thyroid gland and produce more thyroid hormone.

Guillain-Barré syndrome (GBS)

This is an acute inflammatory demyelinating polyneuropathy. The disease is mainly affecting peripheral nerve system. This also listed under autoimmune disorder.

Hashimoto's thyroiditis

This is the first discovered autoimmune disease. Japanese Researcher Hashimoto Hakaru discovered antibodies against thyroid gland. In this disease antibodies formed against thyroid gland will destry thyroid. This disease is commonly misdiagnose as chronic fatigue syndrome, depression, anxiety disorder, etc. hashimoto’s thyroiditis can be diagnosed by testing T3,T4, TSH, Anti-thyroid antibody, Thyroid peroxidase antibody.

Myasthenia gravis

This disease characterized by muscular weakness and fatigability. In MG the acetylcholine receptor mechanism of neuro-muscular junction is affected by circulating autoantibodies and cause impairments in neuro-muscular transmission of impulses.

Narcolepsy

Narcolepsy is a chronic sleep disorder. In narcolepsy patient sleeps while studying, while doing their jobs, sleep in inappropriate time. They will go to REM (Rapid Eye Movement) sleep within 10minutes, but in a normal person it will take about 90 minutes to go to REM sleep.

Pemphigus vulgaris

It is a chronic inflammatory and very painful bullous lesions affecting skin. Autoantibodies form against desmoglein 1 and desmoglein 3, resulting in loss of adhesion between keratinocytes in the epidermis, resulting in bullous painful lesions.

Polymyositis

Polymyositis is infection of multiple muscles of the body.

Primary biliary cirrhosis

This disease characterized by the destruction of small bile ducts of liver. It can be diagnosed by Liver Function Tests, and by identifying the presence of Anti-nuclear antibody and anti-mitochondrial antibody.

Psoriasis

Psoriasis is multi-system autoimmune disease.

Rheumatoid arthritis

RA is very common multi-system autoimmune disorder.  Mainly affects joints, skin, Lungs, Kidney, Heart and blood vessels, etc. Blood test reveals presence of RA Factor, ACCP, but absence of  RA Factor or ACCP does not rule out the Rheumatoid arthritis. Presence of RA factor not confirms rheumatoid arthritis too.  X-ray and other imagine study can also be used for diagnosis of Rheumatoid arthritis.

Sjögren's syndrome

Here autoantibodies attack exocrine gland such as salivary gland and tear gland. Dryness in mouth and eye is the main symptoms. In blood examination ANA (Anti-nuclear antibody) may be seen. In SS ANA patterns are SSB/La.

Systemic Lupus erythematosus

SLE is autoimmune diseases that affect any part of the body. Now a days SLE is a common disease in clinical practice .  Anti-nuclear antibody and Anti-ds DNA Antibody is almost specific to SLE, but it is positive only in 70% of SLE cases, but it will be positive in only 0.5% of people without SLE. American College of Rheumatology established 11 criteria for diagnosing SLE, in which presence of 4 can suggestive of SLE. The 11 criterias are:

1. Malar rash (rash on cheeks).
2. Discoid rash (red, scaly patches on skin that cause scarring).
3. Serositis: Pleurisy (inflammation of the membrane around the lungs) or pericarditis (inflammation of the membrane around the heart).
4. Oral ulcers (includes oral or nasopharyngeal ulcers).
5. Arthritis: nonerosive arthritis of two or more peripheral joints, with tenderness, swelling, or effusion.
6. Photosensitivity (exposure to ultraviolet light causes rash, or other symptoms of SLE flareups).
7. Blood—hematologic disorder—hemolytic anemia (low red blood cell count) or leukopenia (white blood cell count<4000/µl), lymphopenia (<1500/µl) or thrombocytopenia (<100000/µl) in the absence of offending drug. Hypocomplementemia is also seen, due to either consumption of C3 and C4 by immune complex-induced inflammation or to congenitally complement deficiency, which may predispose to SLE.
8. Renal disorder: More than 0.5 g per day protein in urine or cellular casts seen in urine under a microscope.
9. Antinuclear antibody test positive
10. Immunologic disorder: Positive anti-Smith, anti-ds DNA, antiphospholipid antibody, and/or false positive serological test for syphilis. Presence of anti-ss DNA in 70% of cases (though also positive with rheumatic disease and healthy persons).
11. Neurologic disorder: Seizures or psychosis.

Temporal arteritis (also known as "giant cell arteritis")

It is an autoimmune inflammatory disease of vessels of head.

Ulcerative Colitis and Crohn’s Disease

Both are chronic inflammatory autoimmune disease of intestinal tract. The main differences are:-

These are the common autoimmune diseases prevailing in our country. Day by day diseases are falling under the fold of autoimmune disease. One should use his logic to interpret the autoimmune nature of disease before switching over to the treatment. A modern trend is to list all unexplainable health problems under autoimmune disease. An auto immune disease should confirm only by laboratory investigation for an antibody. Scope of homoeopathy shall get an extra millage as new disease comes to the fold of autoimmune diseases.

Impressionable: Readily or easily influenced; suggestible. Capable of receiving an impression. Most of the interpreted Sympathetic shall be impressionable. Sympathetic will respond to the suffering of others in a positive way. Impressionable make it as his grief too but never responds with a deed than word. They will take the impression of sufferings of others to their mind and think that if it occurs to me what will be my feeling? They will share it with others and make them sympathetic but didn’t do anything for which they are letting tears. But a sympathetic person will do something for them. He himself jump into the issue and lead for an action. His deed will be appreciated by others than his words. But the word of an impressionable man will be appreciated by others. 2 Patients reached in my clinic after few days of a bus accident. Both of them were reached from the same rout. One among them was suffering from sleeplessness after seeing the pitiful seen of accident dead bodies, injured people etc. The other person was suffering from IVDP due to lifting people who met accident. He was not able to get treated because he was running after the injured for rescue operations and to console the relatives of dead people.
